cKids First Time Guest Registration

As a part of the consideration for Celebration Church to care for my child(ren), all the named children listed above, I hereby release Celebration Church from any and all claims that might result from leaving my child(ren) in their care. I understand all reasonable safety precautions will be taken at all times by Celebration Church and its agents during events and activities. I understand the possibility of unforeseen hazards and know the inherent possibility of risk. By signing below, I agree not to hold Celebration Church, it’s leaders, employees, ad volunteers liable for damages, losses, or injuries to my child.

By checking in my child for Celebration Church's cKids Ministry, I grant permission for my child to be photographed and/or recorded on the premises (when applicable). I understand that these images and recordings may be used solely for the purpose of promoting and marketing Celebration Church's programs, including but not limited to church publications, social media, and the church website. I acknowledge that my child’s name or personal identifying information will not be included. I understand that no compensation will be given for the use of these images and recordings.
